Property Management Service Request Form

This is an example form for a property management company to use on their website so that tenants can submit Service Requests. This form has been configured so that service requests for each individual property can be routed to the specific property manager in charge of that property. You can also set the form to BCC to the administrative office so that the property management company can receive a copy of all requests in addition to the specific manager on duty. This form requires Gravity Forms and WordPress.
